Specific documents discussed include: the consultation note / history and physical, operative report, admitting orders, operative note, follow-up SOAP note, and discharge summary. This is done through a case presentation format with downloadable templates provided for the viewer's education. Topics also discussed include methods to prevent medical errors through appropriate charting and basic chart rules.
