Selective Neurectomy for Peripheral Neuroma

This presentation focuses on basic surgical maneuvers used to manipulate peripheral nerve trunks, assess their quality and function, including intraoperative electrical activity, and provides a 3-step algorithm for the surgical treatment of symptomatic neuromas in the lower extremity. It also discusses the potential benefits of surgically implanting a temporary, (4 to five days) non-biodegradable catheter for the delivery of local anesthetic solution at the point of neuroma formation, in an effort to predict just how a patient might feel after successful neurectomy. Finally, it details techniques used to contain and relocate a freshly prepared nerve stump, and shows the outcomes of these methods used in the treatment of 76 patients.

Describe the response of the peripheral nerve to various forms of injury, with resultant neuroma formation
Describe the clinical and electroneurodiagnostic methods for making the diagnosis of a peripheral neuroma, and the surgical maneuvers used to treat stump neuromas as well as neuromas in-continuity
Describe a 3-step surgical approach to the treatment of a symptomatic peripheral neuroma, including external neurolysis, endoneurolysis, and excision
