Most pathologic flexible flatfoot deformities are actually cavus feet that due to a host of inherited and acquired etiologic factors are disrupted from maintaining their intended higher-arched architecture upon loading. The collapsible cavus foot is not truly a flatfoot but a flattening foot that has undergone structural failure. Management relies on identification and neutralization of the underlying deficiencies.
