This lecture reviews the principles of off-loading including, total contact, surface area, mechanical, deformity and strain rate. The differences in neuropathic gait, including changes to muscle function and balance will be explored along with the typical gait seen in patients with diabetic foot ulcers. Dr Hanft also covers current modalities and utilization with attention to the “true” functionality and scientific evidence on force reduction. In addition to the review of case studies, current practices, available devices, alternative therapies, and “better” choices are analyzed.
